Look, smarty pants -- I am trying to get minutes done for a meeting, and I don't want to have to re-create a dang attendance list every month -- it'd be delightful to create a form and template it so all I have to do is mark off who was present and who wasn't!

In case whatever rivka linked to didn't work for you as it didn't work for me, right click and go to "Bullets and Numbering", then fiddle around with that until you find what you want. I just hit customized one of the thingers, went to "Characters" and scrolled down until I found a little box that fit my needs. Or rather, your needs. I'd be willing to bet that theres a much easier way to do it, but that should work.
