I've heard (and used) affect as a noun, but only in the context of music. It's related to the doctrine of affects, a baroque concept in which certain musical characteristics (particularly keys) are associated with certain emotions. So, affect in relation to music means "emotional meaning"--not entirely unrelated to effect, but still not the same.

Also, I could be completely off here, but I think it also can be used to indicate affected behavior--as in, behavior that's an act. For example:

Her horror was nothing but affect.

Is this even close to right, or am I just making stuff up?

The only one I missed was the affect/effect one I simply try to avoid using either word in a written sentence so I don't have to deal with the problem. I always second guess myself and change it to the wrong one.

I affect an attitude of not caring about the effect it has on others when I try to affect their awareness of their poor grammar. In effect, I effect no small amount of indignation but I pretend that it doesn't affect me.
