So, the magazine I wish to submit to calls for a cover letter which contains information about myself and any publication credentials I have.
I don't want to cover page to look hokey. Does anyone know what the correct format for a cover page is?

I can't seem to find a good link, so I'll just type out an example here.

Dear Mr. van Gelder,
Please consider my story "Make the Pie Higher" for publication in your magazine.
My past publications include fiction published in Asimov's, SCI FICTION, Black Gate and Lady Churchhill's Rosebud Wristlet. I received the James Tiptree Jr. Award in 2003 for the novel The Past Is Over.
Thank you for considering my submission. I look forward to receiving your reply.
Sincerely,

Something in that format should be adequate. If you do not have publishing credits relevant to the magazine you're submitting to, simply omit them; if you are previously unpublished, say so. There is no need to mention what rights you are offering unless your story is a reprint and you are submitting to a magazine that accepts reprints.
I'm probably missing something out, but I'm sure someone will correct me if that's the case.

I always start out with a statement of my innocence. You shouldn't beat around the bush. Then say that even if you HAD done it, there's not really a problem because it was the right thing to do under the circumstances. Close with a commitment to not rest until you find the guilty party(ies) and ensure that they are brought to justice.
